Perhaps the UK needs a DMP?

Ese mismo sucede con los héroes, de tener héroes perfectos como Superman vamos cambiando a seres oscuros y atormentados como Batman. Podríamos llamar a este proceso la "humanización" de nuestros héroes. Crecemos y podemos tener muchos héroes que van de cantantes a políticos, de jugadores de béisbol a religiosos, de hombres de negocios a personas comunes y corrientes.

Hace ya 7 años, conocí de casualidad a una venezolana, Karina, la Llanerita. Comenzamos a conversar y nos hicimos muy amigos. En aquellos primeros días no tenía el Internet en casa porque en Perú la compañía telefónica todavía no quería dar el servicio, así que me iba un cibercafé, en Perú los llamamos cabinas de Internet, para poder conversar con mis amigos del mundo y con Karina.

Meses después me dijo que tenía algo importante que decirme, que era algo que probablemente haría que me espantara y ya no hablara más con ella. Me preguntaba qué podría ser eso tan grave. Me dijo que usaba silla de ruedas, debido a que tenía DMP (Distrofia Muscular Progresiva) y me preguntó si después de saber eso "tan grave" quería seguir siendo su amigo.

La respuesta evidente era que sí. En este tiempo que hemos compartido he podido conocer a una personita muy alegre y especial, alguien que me ha ayudado mucho con mis problemas, alguien que me ha acompañado con su sonrisa en tiempos difíciles, alguien con quien he compartido mis alegrías, alguien con quien he rezado, alguien que me ha enseñado que la vida es una lucha a la cual se debe enfrentar con toda la determinación que podemos tener.

Determinación y sonrisa. Sonrisa en el alma y en el corazón, aunque no se la pueda expresar físicamente.

En este tiempo, Karina se ha convertido en mi heroína, alguien en quien la lucha del día a día es una manera de vivir, no de sobrevivir. Vivir para la Llanerita es no abandonarse, es sobreponerse a la adversidad de su DMP con todas sus fuerzas y su mejor humor.

Los héroes son más fuertes que todos nosotros, así debe ser y así será siempre.

I'm trying to keep the other things at bay. £700 water bill, £170 court fine, £500 back council tax, £1300 house deposit, packing, arguing with David, Devon's ever present attitude, and then the "how did this happen?" A few months ago things were ok. David made a small bundle selling a picture to the nationals, we started the debt management plan and felt positive about it, I felt like were starting to move forward again.

So, I try not to think about it, any of it, unless I have to.  I make phone calls, and set up payment arrangements and ask questions and then I leave it. I move on. I think I am in a place I have never been in all my life.  I can't really describe it. I just feel that one more tear, one more angry word, one more blow will just tear me apart.

I've been sad before, I've struggled with depression most of my life, but this...this is so dark. This is so oppressive. I know I can't crawl into bed and hide from it, but it's what I want to- desperatley. 

So, I try and not think about it. I think about the party the kids will have in September, once we've moved into the new house, and school has started.  I think about starting school in October, and how excited I am. (But not about how we are going to pay for it. Definetly not.) I think about packing. Not what it means, not all that goes with it, just putting things in boxes, arranging them, labelling them, updating the spreadsheet.

I want to wake up one morning, and things will be better. I just need to get there. I have to get there. It's not an option not to get there. So, we will. We will. We will get there.

I've been saying that for a long time, it seems.

Anders als in der Regelversorgung werden Patienten im DMP regelmäßig bezüglich ihrer chronischen Erkrankung untersucht; die Behandlungsergebnisse werden systematisch dokumentiert. Das gilt auch, wenn es den Patienten gut geht. Die dokumentierten Daten werden unter Berücksichtigung des Datenschutzes für die medizinische Qualitätssicherung und die Evaluation der Programme verwendet.

Um genau diese Dokumentation geht es mir im Folgenden. Zum 1.7. hat es wesentliche Änderungen gegeben. Und diese mußten natürlich in die vorhandene Praxissoftware eingepflegt werden. Vorhandene Software mußte neu zugelassen werden.

Der bürokratische Aufwand wird allenthalben kritisiert. Bislang gab es eine papiergebundene Dokumentation, die von vielen Ärzten tatsächlich per Hand ausgefüllt wurde. Beispielhaft sei die Position der Deutschen Krankenhausgesellschaft aufgeführt, nachzulesen in einer Niederschrift einer Tagung der Landeskrankenhausgesellschaft Brandenburg (siehe unter Anlage 3 im gelinkten Dokument). Dort heisst es:

Der bürokratische Aufwand der Disease Management Programme konterkariert das Ziel einer
besseren medizinischen Versorgung. Die Versorgung chronisch kranker Menschen muss so
weit wie möglich vereinfacht werden. Die Abkoppelung vom Risikostrukturausgleich wäre ein
wirksamer Beitrag zum Bürokratieabbau.

Im Moment jedenfalls sind noch nicht alle Zertfizierungen der Software abgeschlossen, so dass die bereits ab 1.7. notwendige Software teilweise nicht funktionsfähig ist, unter anderem ist eine Abrechnung / Übermittlung der Daten an die zuständigen Stellen ohne diese Zertifizierung nicht möglich, vor allem aber werden die eingegebenen Daten ohne die Zertifizierungs-Codes nicht auf ihre Plausibilität hin überprüft, wie es bislang war.

Wenn diese Codes in den folgenden Wochen endlich zur Verfügung gestellt werden, kommen auf die Praxen erhebliche Mehrarbeiten zu.

There should have been many more settings, not just kill and stun.  Kill, stun, limp: that's the next one down, isn't it?  ...or maybe on "bit of a cough" setting, even lower than that.

Some devices already have a remotely enabled kill switch, such as corporate Blackberries with remote wipe cabability (intended to protect sensitive company data should it be lost or stolen), and others will soon follow, like reports that OnStar is adding the ability to remotely stop the engine of a connected car (again, marketed as an anti-theft system).

Microsoft, however, is looking to set its phasers on stun, limp, or even "bit of a cough."  They've filed a patent application for something they call Device Manners Policies (DMP), another Minitrue-style name and acronym, which, like Digital Rights Management is less about manners (or rights) and more about restrictions.  Schneier calls it Selective Device Jamming.  Essentially, under this scheme, locations will be outfitted with hardware to broadcast to your devices the rules of the land, such as "vibrate only" for cell phones, or "no photography" for cameras.  Hospitals or airplanes where critical equipment can be subject to interference from wireless devces would be able to force your devices into sleep mode until you leave the area (how will such wireless transmissions be guaranteed not to cause interference themselves?).

Microsoft wants to draw analogies with the societal guidelines we call "manners," i.e. that it's considered rude to talk on your cell phone in the movie theatre.  However, this is a false analogy since manners are guidelines, not rules.  DMP wants to disable functionality in your electronics (albeit temporarily) without your consent, or force them into sleep mode: limp and stun settings.

No, an actual manners technology is only a short step away from the "location-based services" stuff that all the cool kids were talking about 2 years ago--some of which are already out.  See, once your devices know where they are, you can do digital manners all client-side, without having to contact the Borg Cube to get your orders.  You have a couple different profiles, such as "theatre" which might mean switching to silent, "office" which sets ring volume to low, and "street" which sets it to high so you can hear it above the sounds of the city.  Simple, no external restrictions, and the user still stays in control.  Each person is free to choose to obey social guidelines or not: just like real manners.

Methods for opting out of some aspects of the surveillance society have been around for quite some time - unlisted numbers, junkbusters, no-call lists, cryptography - and we can expect more in the future.  Unfortunately, this capability cuts both ways.  At Wired, Bruce Schneier details the means by which the surveillance society will be opting you out, whether you like it or not:

OnStar will soon include the ability for the police to shut off your engine remotely. Buses are getting the same capability . . . The Pentagon wants a kill switch installed on airplanes, and is worried about potential enemies installing kill switches on their own equipment.

Microsoft is doing some of the most creative thinking along these lines, with something it's calling "Digital Manners Policies."  Cellphones could be remotely set to vibrate mode in restaurants and concert halls, and be turned off on airplanes and in hospitals. Cameras could be prohibited from taking pictures in locker rooms and museums, and recording equipment could be disabled in theaters.

Most people wouldn't like their own vehicle equipped with a remote kill switch but might think the world would be safer if everyone else had one.  Likewise, we might tolerate our cell phones and camcorders being disrupted in movie theaters if it meant we didn't have to tolerate the boorish behavior of others or sit through another "Piracy Is Bad" trailer.  This is the appeal of DMP: we can reduce the capacity of others to annoy us without confrontation and without taking any personal responsibility whatsoever.  Unfortunately, and as sinister a vision as a nation of self-absorbed xenophobes crouching behind a technologically enforced code of conduct they are too timid to enunciate themselves is, Digital Manners doesn't end there.  That's where it begins.

One of the only ways to maintain liberty in a surveillance state is to surveille the state itself - if the police can record the citizenry, the citizenry must be able to record the police.  The cellphone camcorder with a wireless YouTube connection is an important part of that capability, and its use as a tool against oppression must not be diminished in the name of civility, intellectual property, or Saving The Children.

Fortunately, it probably won't work very well:

If you think Microsoft's vision could possibly be securely designed, all you have to do is look at the dismal effectiveness of the various copy-protection and digital-rights-management systems we've seen over the years.

My cellphone, camera, laptop, and MP3 player might be throwaway items now, but that's likely to change in a Digital Manners future.  One can expect a growing market in the repair and sale of vintage, ill-mannered electronics as well as a black market in DMP-defeating technologies if "Microsoft's vision" comes to pass.

In conclusion, Living In the Future presents three kill switches for the masses: TV-B-Gone, which disables those ubiquitous mind control devices, an RFID-blocking wallet to keep your financial data your own, and an anti-CCTV hat, shown in the lead photo.

Sounds nice, no? Having a chip that temporarily bricks cell-phones in the quiet carriage. But of course, digital manners is the same kind of newspeak as DRM. Just like drm doesn't protect your rights (it limits them), digital manners defies the whole concept of manners.

Not shooting at your neighbour is a matter of law. Not shouting at him is a matter of manners. The distinction is there for a purpose: some aspects of life are left to your own discretion and good judgement. They are enforced by social convention and peer pressure. Regulating such issues means proclaiming that you are not fit to make those judgements. It is acceptable in a nursery or a psychiatric ward, not on the street.

Digital manner is not about manners at all, its about a company thinking it has the right to dictate how you should behave. I wish I could wave it off as bad manners, but its much worse. It takes digital feudalism to new heights.

Google Health est arrivé. A force de rater le lancement du DMP, d'autres l'ont fait. Et visiblement beaucoup mieux... Je suis curieux de voir ce que tout celà va donner dans les années à venir.

Comme attendu, Google Health est un dossier médical personnel géré par le patient et destiné a être partagé avec des institutions et services "partenaires" : hôpitaux (Beth Israel Deaconess Medical Center, Cleveland Clinic), pharmacies (Longs Drugs, Walgreens), laboratoires d'analyses biomédicales (Quest Diagnostics), des "pharmacy benefit managers" (RxAmerica, Medco) et autres services développés autour de la santé (SafeMed, détection de potentielles interactions médicamenteuses, Healthgrades, outils de rating des services de santé).
Ce service est exclusivement réservé aux résidents US, et ne se substitue pas à un avis médical en bonne et dûe forme.
L'intérêt annoncé est celui de la centralisation en ligne et de l'accessibilité permanente de ses données de santé, dans un environnement sécurisé et garantissant la protection de données.

Ce sont bien sûr ces deux derniers éléments qui feront (ou pas) la réussite de ce service.

Blog kali ini seputar multipath software yang puenting untuk diketahui oleh orang-orang IT infrastruktur. Kalau saya amati, hampir 90% server-server saat ini menggunakan SAN untuk kebutuhan datanya. Pastinya server-server ini membutuhkan konfigurasi high availability untuk ketersediaan datanya mulai dari storage controller, switch sampai HBA sehingga failover bisa jalan otomatis jika ada ada failure.

Akan tetapi.. meskipun secara hardware sudah redundant, tapi di sisi server masih perlu multipath software. Otherwise... yaa percuma laah konfigurasi hardware yang redundant itu. Sedemikian pentingnya sampai-sampai beberapa OS sudah menyiapkan solusi untuk multipath ini tanpa memerlukan third party software.

ngapain aja sih multipath software ini, apa kelebihan/kekurangan multipath software bawaan OS dibanding thirdparty software??

Tugas multipath software yang utama adalah menconfigure physical path ke storage menjadi logical path di OS sehingga OS memiliki kemampuan untuk failover atau fallback secara manual atau otomatis. Tugas lainnya adalah load balancing sehingga frame-frame I/O yang dikirim ke storage dapat dibagi-bagi melalui path-path yang ada. Beberapa multipath software yang merupakan feature dari OS sudah memiliki kemampuan failover/fallback dan load balancing namun ada juga yang belum support load balancing a.k.a. hanya active-passive connection saja . Tapi sayang dong investasi yang sudah dikeluarkan mahal-mahal untuk High Availability tapi cuman dibiarkan standby mode saja (active-passive). Oleh karena itu sebaiknya menggunakan multipath software yang sudah memiliki kemampuan load balancing (active-active).

Windows menggunakan MS MPIO (Microsoft Multipath I/O) di W2K atau W2K3 servernya. MS MPIO ini selain support failover dan fallback, juga support loadbalancing.

Di Linux sudah support multipath di kernel 2.6 menggunakan modul DM (Device Mapper). kernel 2.4 juga bisa di support tapi perlu patch. Sementara di 2.6 sudah native bahkan merupakan default di instalasi redhat versi 5. Linux DM ini selain mendukung failover & fallback juga sudah support load balancing menggunakan metode round robin.

Solaris mendukung mutipathing menggunakan MPxIO. di /kernel/drv/scsi_vhci.conf saya tengok sudah support load balancing juga. Sepertinya hanya round robin saja seperti linux.

IBM AIX mengggunakan Multipath I/O di AIX 5L yang juga support load balancing sejak versi 5.2.

HP-UX menggunakan PV-links untuk multipath softwarenya. Sayangnya PV-links masih belum support load balancing (hanya active-passive). Bisa siih di load balance secara manual dengan membagi path LUN2 yang ada ke beberapa HBA. Tapi berdasarkan pengalaman selama ini cara seperti ini tidak optimal. Solusinya bisa dengan menggunakan third party software seperti veritas DMP (Dynamix MultiPath) atau software mutipath bawaan storage vendor seperti HP Secure Path, EMC powerpath, HDS HDLM, atau yang lainnya.

Keuntungan menggunakan multipath software yang native dari OSnya adalah cost. Sudah bundled with the OS koq.. jadi ndak perlu keluar biaya lagi untuk beli lisensi third party software. Tapi kalau bicara performace maka multipath software dari storage vendor mengungguli multipath software bawaan OS tersebut. Karena para storage vendor ini lebih mengetahui isi / konfigurasi storage array mereka sehingga wajar saja kalau mereka bisa optimize metode2 pengiriman packet data ke storage. Contohnya seperti di EMC powerpath, disamping menggunakan basic failover mereka menambahkan option ClarOpt untuk CLARiiON arrays dan SymmOpt untuk Symmetrix (DMX) storage. Tentu saja para EMC engineer lebih tahu kelebihan option2 ini dibanding basic failover untuk masing2 type storage mereka.
Jadi pilih yang mana??.. yang native OS atau third party??.. :)

Bambi and Her Pink Gun, by Kaneko Atsushi (DMP): Dystopia populated by creepy, violent societal parasites? Check. Unsympathetic protagonist who cuts a giddy swath through their ranks? Double check. DMP dropped this delightfully nasty series after only two volumes, but oh, those two volumes are filled with cheerful misanthropy. I miss Bambi.

Dragon Head, by Minetaro Mochizuki (Tokyopop): You’ve probably said it to yourself: “If I see one more school trip in a manga series, it had better end really badly.” This is the manga for you. In ten volumes of pretty much relentless terror, with occasional side trips to mere creepiness, a handful of survivors try and figure out what the heck happened to Japan while their train was passing through a tunnel.

The Drifting Classroom, by Kazuou Umezi (Viz): If you could harvest the terrified screams of children and use them as an alternative fuel source, you could probably use this book to power the Mid-Atlantic Region for a few months, at least. I know I shouldn’t admit that watching elementary school children meet grisly and varied ends is a real hoot for me, but it is.

MW, by Osamu Tezuka (Vertical): Tezuka is generally an optimist, but that doesn’t mean he’s naïve, or that he can’t be downright depraved when the situation calls for it. MW calls for it over and over, and Tezuka doesn’t shrink from any of the lurid possibilities of kidnapping, mass murder, blackmail, illicit sex, and so on.

Uzumaki, by Junji Ito (Viz): “Uzumaki” means “spiral,” as in “downward.” The third volume of Viz’s re-release of this grimly imaginative horror series is probably already available in bookstores, or you can wait until next week when it’s due to show up at the comic shops.

Antique Bakery, by Fumi Yoshinaga (DMP): Okay, it’s more about coping with the challenges of adulthood in general than romance in particular, but I think Yoshinaga is at her funniest, sharpest, and most generous when she examines the bittersweet qualities of interpersonal relationships. It’s almost all sighs instead of swoons, but a story doesn’t have to offer anything resembling “happily ever after” to be romantic in its own way. All four volumes are available.

Emma, by Kaoru Mori (CMX): On the other hand, this one is all swoons, all the time, and it is glorious. It follows the fraught-with-obstacles romance of a housemaid and a member of the upper class (though tellingly, not the aristocracy), rendered with breathtaking emotional precision and lush, detailed illustrations. Only one more volume is due from this series.
