<p>As fans are highly expecting the newly forth coming album this fourth quarter, fans are also in for another treat. Fans can also expect an autobiograpy by the Detroit rapper which will be released this month entitled "The Way I Am".</p>
<p>This Book will feature photo's of Eminem's home and original drawings, as a way of broadcasting his initiatives through his creative process. The Way I Am will be loaded with a reflection of Em's greatest hits through his career as an artist. This book is compared and praised in the same ranks as Tupac’s The Rose that Grew from Concrete, Bob Dylan’s Chronicles, and Journals by Kurt Cobain. </p>
<p>The book will hit stores everywhere on Oct. 21st so be on the lookout for that.</p>

Well, let me explain a couple things.</p>
<p>My name is Mr. Modern, (Well, not really) and I am not an adult. I am an average teenager living in the suburbs.</p>
<p>So why do you care?</p>
<p>I am the kind of guy that wants to publish myself on the web, and be a help, and a service to people.</p>
<p>So I decided to create a Blog.</p>
<p>This blog is about modern trends, as I observe them, and what to know, so you are in the know.</p>
<p>This blog is for anyone interested in knowing what's what in today's world, according to an average teen.</p>
<p>So to start off with, let me explain who I am.</p>
<p>I am a teenager who lives within a suburban town, and I watch and observe everything. I am known to be extremely tech-savvy, and with this savvy-ness I hope to answer any questions you have.</p>
<p>I am not one of the popular kids, but they all know me, and say hi from time to time. I see and observe and discover things I feel a normal teenager would not notice or even care about. Things about trends, social patterns, and other things.</p>
<p>This blog is to share these discoveries with you, giving you examples (with changed names of course) of instances where I observed these happenings.</p>
<p>I don't care what you think of me, my blog, my writing skills, or my dramatic way of expeessing myself, I do care about your problems, your questions, and your curiosities regarding the teenage world, technology, and culture.</p>
<p>So I am here to help. Please ask questions about anything, and I will try to answer them to the best of my ability.</p>

<content:encoded><![CDATA[<p>The weather here in Southeastern Virginia has been absolutely gorgeous.  Highs in the 70s, light autumn breezes, bright sun shining from the clear blue sky.  Perfect weather to have all the windows open, to consider cooking and baking, to nap in the breezes.</p>
<p>Instead, I went and had a <a href="http://en.wikipedia.org/wiki/Panic_attack">panic attack</a> yesterday.  It was my very first, full on, thought-I-was-going-to-die panic attack.  I was nauseated (word to the wise: <em>nauseous</em> means you're full of vomit; <em>nauseated</em> means you feel like you're going to vomit.  just sayin'), I couldn't get a full breath in, I felt my heart racing, I was burning hot, my temperature registered at <a href="http://en.wikipedia.org/wiki/Hypothermia">96F</a> (about 35.6C, or possibly lower, since I didn't shake the thermometer down below the 96 mark), I was trembling all over, and I felt too weak to move my body.  At 6:30 pm, I put myself to bed, called poor maplestar, and told him things like who has keys to the house and where my living will paperwork is.  Yeah, I was <em>that</em> scared.  If by <em>scared</em>, you're thinking <em>absotively posilutely terrified</em> that this was my last night on earth.  As a measure of this fear, I locked only the doorknob lock, and not the deadbolt lock, and I didn't set the alarm system before I put myself to bed.  I thought I was freakin toast.</p>
<p>Thanks be to God, I wasn't.  (Obviously, or you wouldn't be reading this.)</p>
<p>I know what triggered it.  It was a combination of events that led to me feeling completely, one-hundred-percent abandoned.  Yes, I knew I could count on maplestar, and I do count on him heavily, but he is seven hundred miles away.  Yesterday, I needed a real live person, with the skin on.  Someone to sit with me, to talk with me, to help bring me out of the pain and the misery, to help me find some of the resources I need.  I know that there are people in town who will come to your house to cut your hair, but I don't know how to find them in the yellow pages.  I know that there are other resources available, but I don't know how to find them.  I could really use a case manager to help me navigate the system, but I don't even know how to do <em>that</em>!</p>
<p>So I was alone at home, and it felt like I'd acutely injured my left hip - as opposed to a flare-up of chronic pain, and yes, I can tell the difference - so that sitting made it worse, standing was okay for about five minutes but then made it worse, and the only thing I could to <em>not</em> make it worse was to lie on my right side.  Of course, that didn't make it any <em>better</em>, just kept it from getting worse.  I put in a call to the orthopedist's after-hours number and left a message.  I tried to call a friend from church but just got voice mail, so I sent her an email in which I wasn't quite specific or assertive enough.  She responded in a very sweet or thoughtful way, and told me how crazy-busy her day was, but that I was in her prayers.  Well, at that point, I didn't want any <a href="http://www.nationmaster.com/encyclopedia/List-of-Firefly-curse-words">gorram rutting</a> prayers, I wanted a <em>person</em>.  Then the doctor called back, listened to a very quick synopsis of my general condition and my current symptoms, and said that it was clearly a lower-back issue causing this.  When I told him that I know that all of my <a href="http://en.wikipedia.org/wiki/Cervical_vertebrae">cervical</a> and <a href="http://en.wikipedia.org/wiki/Thoracic_vertebrae">thoracic</a> vertebrae have unstable <a href="http://en.wikipedia.org/wiki/Facet_joint">facet joints</a>, I could pretty much <em>hear</em> him nod as he said, "Given that and your Ehlers-Danlos, it's probable that your <a href="http://en.wikipedia.org/wiki/Lumbar_vertebrae">lumbar vertebrae</a> are unstable, too."</p>
<p>He suggested taking OTC ibuprofen.  I laughed - most likely with a bitter edge - and said that right now I'm taking <a href="http://www.rxlist.com/cgi/generic/propoxnap.htm">darvocet</a>, <a href="http://www.rxlist.com/cgi/generic/etodolac.htm">lodine</a>, <a href="http://www.rxlist.com/cgi/generic/metax.htm">skelaxin</a>, and <a href="http://www.rxlist.com/cgi/generic/cyclobnz.htm">flexeril</a>.  After making sure I don't take the skelaxin and flexeril at the same time, he laughed (not so bitterly) and said, "Well, it certainly sounds like you're prepared!"  He recommended rest, and alternating ice and heat.  (Yawn - now tell me something I <em>don't</em> already know!)  And of course, "If it gets terrible, you can go to the ER."  Um, yeah.  Since I can't even SIT UP, that's remotely possible!  Not to mention that I'd have to get the pain management doctor's permission to take any meds I was given at the ER, since I'm under a narcotics contract for <em>her</em> protection!  He did invite me to call the office Monday and ask for an emergency appointment.  But until then, I was stuck.  At home, alone.  In more pain than I can remember since childbirth.  Without knowing how I'm getting to the doctor and the physical therapist for the next few weeks, since it's awfully hard to drive with a boot on my right foot.  With my parents a few hundred miles away at a Celtic festival, selling their jewelry.  Without the friend from church who I'd really wanted to sit with.  With maplestar <a href="http://hedwyg.wordpress.com/2008/10/04/when-the-internet-doesnt-shrink-distances-enough/">seven hundred miles away</a>, trying to figure out how to help me.</p>
<p>So: panic attack.  Thankfully, I got myself into bed, and despite my worst fears, I managed to give hints to my beloved maplestar that I was indeed planning to talk with him again.  I slept for a couple hours, then woke feeling much more like myself again.  I got up, called maplestar again, ate a little bit, took my bedtime meds, and went back to bed.  I did have some terrible nightmares this morning, probably after most of the meds had worn off, but I slept until almost 7 (which is not bad for me) and lazed in bed until just after 9.  Little TomToms actually came up onto the bed with me, sniffed my fingers, and consented to be petted.  After a few minutes, he was curled up on my lap, purring madly.  He even fell asleep there for a moment.  It was wonderful, and the sad part is, this means he's ready to go to his forever home.  I'll miss the sweet little boy, even if he was very malodorously and frequently flatulent in the half-hour or so he allowed me to snuggle with him.</p>
<p>This weekend, I took a $250+ trip to the supermarket; made an enormous batch of marinara sauce, which has been portioned out and frozen; made an enormous batch of cream of potato soup, which has also been portioned out and frozen; sectioned some grapefruits and stored them with splenda in the refrigerator for snacks; washed several pounds of grapes for munching; and mixed up a ginormous salad in <a href="http://www.amazon.com/Rubbermaid-7J92-Produce-Storage-Container/dp/B000WEIICG/ref=pd_bxgy_k_text_b">a produce saver</a>.  The idea is to have on hand, and ready to eat in a short time, some of the things I like to make for myself.  I've already enjoyed two salads, a bowl of ravioli with marinara sauce, and TWO big bowls of potato soup topped with cheddar and bacon and green onions.  Mmmmm!  Now to see if I (a) remember I have this stuff, and (b) actually warm it up and eat it.</p>
<p>While I was panicking last night, I asked maplestar to email my ride to church and our Christian Ed director to let them know that with the hip all wonky, I wouldn't be able to make it to church this morning.  I mean, it hardly seems appropriate to lead a discussion group while <a href="http://www.flickr.com/photos/12736086@N08/1321747347/">reclining on one's right side</a>.  So after I got out of bed and tended to the kittehs and the birdehs, I had a fruit cup and then sang some hymns at the piano.  It was really nice, and it got me motivated to move on with my day.  I regret missing church, but it was a good, healing morning.  Now, I'm enjoying the cooler evening breezes through the windows, and thinking about some of that leftover ravioli - perhaps topped with mozzerella and baked?  and with some garlic bread, too? - for supper.</p>
<p>I know I am not alone.  I know I have not been abandoned.  I know that I am loved and wanted and blessed.  I'm glad I'm still alive.  And I'm glad (and so lucky!) that maplestar loves me.</p>
<p>May you be blessed tonight with the knowledge that you, too, are loved and wanted and blessed.  May you find deep joy and gladness in your the fact that you are alive.  And may you know that whatever may happen, you will never ever be abandoned.</p>

He's a less talented Clinton, a poll the public and vote that way kinda guy. He just sort of tries to be 'hip'. But he can't even claim <em>that</em> any more. <title><![CDATA[OARSI recommendations for the management of hip and knee osteoarthritis, Part II: OARSI evidence-based, expert consensus guidelines]]></title>

<p>PURPOSE: To develop concise, patient-focussed, up to date, evidence-based, expert consensus recommendations for the management of hip and knee osteoarthritis (OA), which are adaptable and designed to assist physicians and allied health care professionals in general and specialist practise throughout the world.</p>
<p>METHODS: Sixteen experts from four medical disciplines (primary care, rheumatology, orthopaedics and evidence-based medicine), two continents and six countries (USA, UK, France, Netherlands, Sweden and Canada) formed the guidelines development team. A systematic review of existing guidelines for the management of hip and knee OA published between 1945 and January 2006 was undertaken using the validated appraisal of guidelines research and evaluation (AGREE) instrument. A core set of management modalities was generated based on the agreement between guidelines. Evidence before 2002 was based on a systematic review conducted by European League Against Rheumatism and evidence after 2002 was updated using MEDLINE, EMBASE, CINAHL, AMED, the Cochrane Library and HTA reports. The quality of evidence was evaluated, and where possible, effect size (ES), number needed to treat, relative risk or odds ratio and cost per quality-adjusted life years gained were estimated. Consensus recommendations were produced following a Delphi exercise and the strength of recommendation (SOR) for propositions relating to each modality was determined using a visual analogue scale.</p>
<p>RESULTS: Twenty-three treatment guidelines for the management of hip and knee OA were identified from the literature search, including six opinion-based, five evidence-based and 12 based on both expert opinion and research evidence. Twenty out of 51 treatment modalities addressed by these guidelines were universally recommended. ES for pain relief varied from treatment to treatment. Overall there was no statistically significant difference between non-pharmacological therapies [0.25, 95% confidence interval (CI) 0.16, 0.34] and pharmacological therapies (ES=0.39, 95% CI 0.31, 0.47). Following feedback from Osteoarthritis Research International members on the draft guidelines and six Delphi rounds consensus was reached on 25 carefully worded recommendations. Optimal management of patients with OA hip or knee requires a combination of non-pharmacological and pharmacological modalities of therapy. Recommendations cover the use of 12 non-pharmacological modalities: education and self-management, regular telephone contact, referral to a physical therapist, aerobic, muscle strengthening and water-based exercises, weight reduction, walking aids, knee braces, footwear and insoles, thermal modalities, transcutaneous electrical nerve stimulation and acupuncture. Eight recommendations cover pharmacological modalities of treatment including acetaminophen, cyclooxygenase-2 (COX-2) non-selective and selective oral non-steroidal anti-inflammatory drugs (NSAIDs), topical NSAIDs and capsaicin, intra-articular injections of corticosteroids and hyaluronates, glucosamine and/or chondroitin sulphate for symptom relief; glucosamine sulphate, chondroitin sulphate and diacerein for possible structure-modifying effects and the use of opioid analgesics for the treatment of refractory pain. There are recommendations covering five surgical modalities: total joint replacements, unicompartmental knee replacement, osteotomy and joint preserving surgical procedures; joint lavage and arthroscopic debridement in knee OA, and joint fusion as a salvage procedure when joint replacement had failed. Strengths of recommendation and 95% CIs are provided.</p>
<p>CONCLUSION: Twenty-five carefully worded recommendations have been generated based on a critical appraisal of existing guidelines, a systematic review of research evidence and the consensus opinions of an international, multidisciplinary group of experts. The recommendations may be adapted for use in different countries or regions according to the availability of treatment modalities and SOR for each modality of therapy. These recommendations will be revised regularly following systematic review of new research evidence as this becomes available.</p>

<content:encoded><![CDATA[<p>Recently I found myself admiring things that are called 'hip'.  Moreover, I grew to admire the people doing the calling.  They prefer not to be known as anything in particular. However they can be identified by both their following and their detractors as "hipsters".  Hipsters belong to an unofficial network of bloggers, journalists, and niche aficionados who search and scour the globe, and the web for new and progressive culture.  They can be stereotyped by their urban style, the music they do and<em> don't </em>listen to, their zeal for the unique in all areas of life.  They receive praise for their creative minds and passion for a counterculture that restores the human element to all areas of life.  Hipsters collectively attach this buzzing adjective by their actions to the things that resonate loudly from their pitchfork of cool.</p>
<p>More than a year ago I stood squarely outside this subculture and admired all of its trends without understanding the heart of it.  Shortly thereafter I became a trendspotter; one who thoughtlessly adopts whatever is projected from the collective center of cool-dom.  I was a cool consumer.  I loved the idea of being praised by my new found soul-mates and subconsciously loathed the praise that came from those who clearly were not.  I failed to understand that the heart of this movement is led by those who desperately want to rid their lives of the mechanical, plastic, and meaningless output of mainstream culture.</p>
<p>These leaders are neither consumers nor purveyors of a new and attractive trend.  They are generators of the truly cool.  James Deans in their own right.  Their passions are not driven by the always changing trends of anti-establishment nor the goal to be on the cutting edge. In fact, they probably hate being identified by their commercialized drones.</p>
<p>I was a drone.  I was on the edge of my toes watching the horizon for the next thing... before it got big.  Then I noticed an attitude of discontentment quickly coming from all of my latest "discoveries".  This is coming from a person who believes that Jesus Christ satisfies all of the soul's desires.  Don't get me wrong, He does.  However, like anyone with a heartbeat, my own sinfulness got the best of me.  I was driven to find the next thing that would temporarily give me the feeling of being "on top" which I somehow wrongly associated with being content.</p>
<p>This movement is <em>not </em>all bad. I believe their are many leaders within this subculture who, with their passion and creativity, are beginning create inroads which have the capacity to demonstrate the gaping holes in a consumer-driven society.  I pray that we, as believers in Christ, would herald this strength, follow in their path and be bold enough to lovingly share that it is Christ who fills these gaping holes. Not as a satisfying object of consumption, but as a personal loving God, who is glorified most by our reveling in His glory.</p>
<p>In the end, I believe that the spirit of ingenuity that follows those who aren't afraid to march to the beat of their own drum is a good thing.  The burden for me, is to watch that I am not blindly following any drummer, but carefully considering which, if any, of the beats I hear I should follow.</p>

The Minister wore white. The white men wore bindys (3rd eye jewelry normally only worn by women) because they had no idea they weren’t supposed to.  But, the most non-traditional wardrobe choice by far was the purple, velvet gown worn by my nephew.</p>
<p>I recall the day my brother told me that five year-old Calum would be wearing a dress to my wedding. Danny matter-of-factly informed me “when we told Calum the wedding would be comprised of a family parade down the aisle, followed by a party, he said he wanted to wear a suit for the parade and a dress to the party”.   He then explained that Calum thought boy’s clothes were much to boring for dress-up parties and the only solution was to borrow from the girl’s side of the closet with its exciting array of colours and styles.</p>
<p>In truth, it barely surprised me because my brother and his partner had always been very permissive parents.  While most would be rushing their sons to hockey camp or some sort of deprogramming, Danny just told the family that it was best not to pay too much attention to these things and that it would ‘run its course’.  Having lived in the West End of Vancouver for more than a decade, I knew that feminine attire has a very long run in some men’s wardrobes.</p>
<p>In fact, Danny was right and this is exactly what happened. Four years later, Calum wouldn’t be caught dead in purple, velvet or a gown.</p>
<p>But, on a hot May night in 2004, a little boy pushed at least a few people’s buttons by proudly donning a gorgeous grape, Victorian floor-length dress with lace collar and cuffs. This was no kilt, no manly Hawaiian grass-skirt, no spiritually excused holy man’s wrap. No, this was a very simply, a boy in girl’s clothing.</p>
<p>Knowing that the majority of my friends and family were either very hip, very queer or very Canadian – I knew that no one would say anything even if they were bothered. That is, with the exception of my Grandmother who is none of these things.  And so, I did fret a fair bit about the moment that Nana would see her great-grandson in drag.  Being by far the most cultured in the family and the most concerned about appearances, I knew that although she would say nothing at the wedding, I would be hearing about it afterward.</p>
<p>The following day, we gathered for a family brunch and I was seated next to Nana.  After a few niceties she leaned close to me with a scowl and quietly asked “Did you see what Calum was wearing?”  She didn’t wait for my response. Shaking her heard back and forth with a truly sour look she said “I can’t believe he was wearing velvet after Easter.” and turned back to her tea cup, delicately lifting it with her pinky just so.</p>

<content:encoded><![CDATA[<p>Well... the time has come.  Tomorrow morning at 10:00 am CST, I will be undergoing hip surgery.  I have what they call a hip impingement and the only option I have left is surgery.  I have been dealing with the pain for over a year now and it has started to affect daily activities.  After the surgery is complete and assuming there are no complications, I am looking at about 6 weeks of non-weight bearing activity and all kinds of other good stuff I'm sure.  So why am I telling you this?  Well, I was hoping you all would send me all kinds of get well stuff and........ NO, I'm kidding!  I just wanted to let you know you may not see any new posts for a couple weeks while I get my bearings that's all.  It may be a little while before I can go get any cards but I will still be posting rest assured.  I will post a "progress report" as soon as possible and let everyone know what's shakin' - until then may all your busts be big and your hits true.</p>

He was amplifying his art history classes, learning to ‘Paint’ his way. He learned in depth about Kasimir Malevich, of whom he said, “…we never heard of <em>him</em> at Twickenham.” He also really liked the idea of the Russian’s Agitprop fold out stages on trucks and traveling theater. As Kathy Moon says, "He didn't judge, he looked to see what's there."</span></p>

<p class="MsoNormal"><span><span><span>Fulcher watched ITV, the Hanna Barbara cartoons especially, he followed the TV ads; and he liked that bright, flat, Western cowboy-artist in the ‘Boys Annual’ books, and then there was the deeper side to his interests…</span></span></span></p>
<p class="MsoNormal"><span><span><span>One of our teachers was the sculptor Dow, bearded in the manner of his mentor, lean type designer Eric Gill, or an even a better likeness, Ezra Pound. Dow helped introduce us to Ginsberg's Howl, to Burroughs and ultimately the idea of an alternative reality. You can see Dow's version of Eric Gill's deco angular style above what are, or were, the Jaguar showrooms in Piccadilly close by the Green Park tube station. Up in our aerie in the tower, in Mr. Dow’s class in cardboard construction, Dow told us about making the model for the planning of the WW2 Möhne ‘Dam Busters’ raid (see the movie) at the National Physical Laboratory in Teddington, cutting contour relief levels in mounting board with a scalpel. He intones, “One cut has to do it. Your knife must be sharp.”</span></span></span></p>
<p class="MsoNormal"><span><span><span><strong>… to Howl</strong></span></span></span></p>
<p class="MsoNormal"><span><span><span>Mr Dow often talked of life in a way us suburbanites had only read about. One day, a Thursday, back from documentary drawing at the Victoria and Albert Museum by bus, Colin and I are talking Burroughs and the Beats; we’d both read Ginsberg’s Howl. </span></span></span></p>
<p class="MsoNormal"><span><span><span>Always first with hip, Fulcher had lent me his copy. He’s sitting on a stool, by the window behind a bench, first back from doc drawing at the V and A, by bus, late afternoon. I had thought there was a scene of a hanging and ejaculation in Howl, but I’m wrong, must be Burroughs somewhere. Fulcher was inspired by that, is all “Ultimate!” Later, Dow, is chanelling<span> </span>Ezra Pound, who wrote, “I have always thought the suicide should bump off at least one swine before taking off to parts unknown.” Rambling on in conversation, Dow goes, “… and I’m gonna end it when I’m sixty.” Colin says to me afterwards, “I’m going to end it when I’m forty. We should have done it all done by then… Who wants to be old?” I often wonder if Dow, like Barney, followed through on his suggestion.</span></span></span></p>

<title><![CDATA[Is my aching back the same as my neighbor's?]]></title>

<content:encoded><![CDATA[<p>I gave a talk on back pain the other evening and it was more enlightening for me than for the individuals that braved the stormy weather to attend.  When I discussed the types of exercises and treatments appropriate for each type of back pain, one of the men stated that he had a bulging disc at L5 -SI, which is fairly common.  He also stated that he had pain radiating down to his left leg which had gotten better with a spinal injection.  We then discussed what the current physical therapy treatment protocol was and I was appalled. </p>
<p>The gentleman in question was performing an exercise program called Williams Flexion Exercises.  They include bringing up each knee to the chest. sit ups, straight leg raises and a few other exercises that promote bending forward.  These were the exercises that I was originally taught in school about 30 years ago.  Although some of them are appropriate for those patients with stenosis or the narrowing of the spinal canal, they are probably the worst possible group of exercises to be used for a protruding posterior disc at L5-S1.  Those bending forward exercises actually push the disc further out into the space that the nerves are locating potentially causing more leg pain. </p>
<p>All back pain is not the same and should not be treated as if it is so.  A herniated disc at the lowest level of the spine is quite different than a muscle strain, a stenosis, or even a facet problem.  Each needs a different set of exercises and possibly a different set of treatments in the clinic.  If muscles are found to be tight, we stretch them.  If muscles are found to be weak, we strengthen them.  But each individual needs to be evaluated and treated according to the findings in the exam. </p>
<p>There is a small place for Williams Flexion Exercises minus the crunches in back pain treatment.  Most patients need to stay away from those crunches when strengthening the abdominals or the core muscles because they do more to strengthen the hip flexors or muscles that bend the hip than anything for that pouchy stomach.  Bending forward exercises are usually prescribed for stenosis.  They help to open up the spine.</p>
<p>Most lower lumbar discs respond better to extension exercises described by McKenzie.  Those are typically performed in the prone or laying on the stomach position.  I also usually prescribe strengthening the core muscles in a neutral spine position.  That means that you want to decrease the curve in the lower spine if it is too flat or to increase that curve if it is too rounded.  In any case, no two backs are the same.  Each one needs evaluation and specific treatments to address the problems found. </p>
<p>This gentleman was obviously seeing a physical therapist or chiropractor who was not up to date (within the past 15 years) on his treatments.  If you go to someone's office for back pain and you are treated with only hot packs, electrical stimulation and ultrasound with these types of exercises including crunches, it is time to find a more up to date practitioner.  There is so much more to good back treatment today. </p>
<p>What should one look for?  Start with a practitioner who performs a thorough evaluation.  Then he or she should be able to explain what the specific problems are and how he or she plans to address them.  Finally the treatment should include some type of hands on treatment and some things you can practice at home.  I expect my patients to feel some relief after each treatment when any type of pain related problem is involved.  Strengthening and stretching muscles as well as education patients about posture may take some time. </p>
<p>All back pain is not the same and should be treated as such.  Every aching back is potentially a completely different problem.  So is my aching back the same as my neighbor's?  The odds are against it.  You want to find a physical therapist who takes the time to figure out what is best for your particular problem.</p>
