et article tente de faire le tour du sujet, via le web, à l'automne 2008. Ce n'a pas été simple, même pour un pro de Google : l'information officielle est rare, fragmentaire, et pour tout dire quasi inexistante comparativement aux enjeux de santé publique (présence des pesticides dans l'air, dans l'alimentation, dans l'eau). Il n'y a pas que les abeilles qui trinquent.

Quelques faits, pour commencer :

  selon la MSA, ce serait près de 20 % des agriculteurs et ouvriers agricoles qui se plaindrait de problèmes de santé suite à un traitement : soit environ 100 000 agriculteurs par an ! Les symptômes les plus fréquents d'intoxication sont les troubles cutanés, les troubles digestifs et les troubles neurologiques.

• En 2006, le tribunal de Bourges (Tass) a reconnu la maladie de Parkinson d’un salariĂ© agricole comme maladie professionnelle. La MutualitĂ© Sociale Agricole (MSA) n’ayant pas fait appel, cette dĂ©cision est dĂ©sormais valide. D'autant que les chiffres de l'Ă©tude « Partage » sur la maladie de Parkinson commanditĂ©e par la MSA Ă  l'Inserm sont alarmants : les agriculteurs ont un risque multipliĂ© par deux par rapport au reste de la population d'ĂŞtre touchĂ© par cette maladie.
• Selon les premiers rĂ©sultats de l'Ă©tude CEREPHY (voir ci-dessous), le risque d'ĂŞtre atteint d'une tumeur cĂ©rĂ©brale est 2,6 fois plus grand pour les personnes les plus exposĂ©es aux pesticides.
• L'Ă©quipe du Professeur Sultan (CHU Montpellier) au cours d'une enquĂŞte Ă©pidĂ©miologique rĂ©alisĂ©e en 2002, elle a constatĂ© qu'un enfant d'agriculteur a 4 fois plus de risque d'avoir une malformation gĂ©nitale. L’exposition pourrait provenir du travail agricole des parents ou de contaminations domestiques de l’habitat ou de l’alimentation.
• En France, le pĂ´le santĂ© du TGI de Paris instruit, depuis 2003, les plaintes recueillies parmi les militaires victimes de troubles et ayant servi au sein de la division Daguet durant la guerre du Golfe. Le rapport avec les agriculteurs ? Après avoir soupçonnĂ© les munitions dopĂ©s Ă  l'uranium, le pĂ´le santĂ© s'intĂ©resse aux travaux du Dr Pello, qui Ă©tudie depuis douze ans les effets des pesticides sur le corps humain, constatant que ses patients, principalement des agriculteurs dans la force de l'âge, se plaignent de fatigue chronique, de douleurs musculaires ou de troubles de la mĂ©moire. Les symptĂ´mes dĂ©crits sont les mĂŞmes que ceux observĂ©s, selon lui, chez les vĂ©tĂ©rans du Golfe soumis eux aussi Ă  des doses massives d'«organo-phosphorĂ©s», une des trois classes de pesticides...
• La France est le deuxième utilisateur mondial en quantitĂ© de pesticides utilisĂ©s, après les Etats-Unis, avec près de 100.000 tonnes de pesticides (ou produits phytosanitaires) utilisĂ©es chaque annĂ©e en milieu agricole, auxquels s'ajoutent les produits Ă  usage domestique ou de loisir. La population exposĂ©e est nombreuse avec 664.000 exploitations agricoles, correspondant Ă  une population active familiale de 1.155.000 personnes employant environ 154.000 salariĂ©s permanents, plus les jardiniers occasionnels. Les produits sont divers avec 9.000 produits pesticides commercialisĂ©s en France ce qui reprĂ©sente environ 900 substances actives. Les agriculteurs sont plus touchés par certaines pathologies

Les maladies neurodégénératives comme les maladies de Parkinson et d'Alzheimer semblent frapper davantage les agriculteurs que le reste de la population.

Le pouvoir cancérigène des pesticides est encore mal identifié. Les effets sur la santé sont bien connus pour les intoxications aiguës, mais mal connus pour les expositions modérées ou prolongées. Trois effets potentiels ont déjà été identifiés par des études épidémiologiques : cancers, troubles neurologiques chroniques et troubles de la reproductionLes études menées en Amérique du Nord et en Europe du Nord montrent que la mortalité par cancer, tous cancers confondus, est moins élevée chez les agriculteurs que dans le reste de la population (en raison notamment, d’une moindre consommation tabagique),. Mais très peu d'études ont été menées en France sur les cancers en milieu agricole. Certains cancers seraient plus fréquents : hémopathies malignes (leucémies, lymphomes malins, myélomes…), cancers cutanés, sarcomes des tissus mous, cancers de la prostate, cancers gastriques et cancers cérébraux. Un accroissement du risque de cancer chez les enfants exposés à des pesticides avant la naissance ou pendant l’enfance est souvent évoqué. L’exposition des enfants aux pesticides a été le plus souvent associée aux tumeurs cérébrales et aux leucémies.

L'Ă©tude CEREPHY en Gironde

Les cancers du cerveau sont rares, mais graves avec 2 800 décès annuels en France. Selon les résultats préliminaires de l’étude française (CEREPHY) de l'ARC réalisée en Gironde, une région viticole utilisant de grandes quantités de pesticides, l’exposition à des pesticides favorise les tumeurs du cerveau.

Selon les premiers résultats, le risque d'être atteint d'une tumeur cérébrale est 2,6 fois plus grand pour les personnes les plus exposées aux pesticides (et même de 3,2 fois plus pour les gliomes, des tumeurs de mauvais pronostic) ainsi que pour celles qui traitent leurs plantes d'intérieur avec des produits phytosanitaires.

«La plausibilité que les pesticides favorisent le développement de tumeurs cérébrales est suffisamment forte pour que des actions soient entreprises», soutient le P^r Goldberg de l'ARC. En revanche, il est impossible à ce jour d'identifier la ou les molécules cancérigènes en cause. D'où la nécessité de poursuivre des travaux dans ce domaine. De telles données restent à confirmer par des enquêtes sur de plus larges populations dans d'autres départements, avec des types de cultures différents. Mais elles mettent l'accent sur le rôle potentiellement nocif de certains produits phytosanitaires.

L'Ă©tude PESTEXPO

L'ARC et de la FNATH ont par ailleurs lancé l'étude PESTEXPO afin de mieux connaître les expositions réelles des utilisateurs au cours d'une journée, d'une saison et d'une vie professionnelle, en fonction des types de culture, des périodes et des zones géographiques. Près de 200 personnes utilisatrices de pesticides ont été observées à la fois dans des grandes exploitations agricoles et de polyculture-élevage dans le Calvados que dans des exploitations viticoles de tailles variées en Gironde. 8.000 dosages ont déjà été effectués sur les tenues de travail. Les paramètres principaux identifiés sont, par ordre décroissant : la phase d'exposition, le type de matériel utilisé et la protection individuelle. Les résultats doivent encore être analysés et aucune présentation n'est prévue pour l'instant.

La MSA, organisme de sécurité sociale de l'agriculture française, se préoccupe du risque "Phytosanitaire"

La première approche de prévention : PHYT'ATTITUDE

La MSA a commencé à s'intéresser au problème sous l'angle des accidents du travail liés à une intoxication par des substances chimiques. C’est pourquoi l'organisme a mis en place un réseau de toxicovigilance agricole, Phyt’Attitude, pour évaluer ces effets et les prévenir. Elle a mis en place un numéro vert pour recueillir les témoignages d'agriculteurs. Les symptômes les plus fréquents d'intoxication sont :

• Les troubles cutanĂ©s : irritations, rougeurs, brĂ»lures et prurits.
• Les troubles digestifs : nausĂ©es, vomissements et douleurs digestives. Une atteinte hĂ©patique toxique a Ă©tĂ© recensĂ©e.
• Les troubles neurologiques : les cĂ©phalĂ©es.

L'étude AGRICAN : premiers résultats fin 2008 ?

Cependant il n'existe pas encore en France d'une véritable étude d'envergure sur les cancers des agriculteurs. Somme, Tarn, Vendée, Gironde, Cote d'or) et d'un questionnaire adressé à plus de 600 000 personnes, afin de couvrir tous les types de population agricole et tous les types de cancers. L'enjeu est donc d'établir une meilleure connaissance des risques liés à l'utilisation des produits phytosanitaires, de leur impact sur la santé, étant données les contraintes d'utilisation et les conditions de travail des agriculteurs. "Nous partons d'un constat : certains cancers semblent toucher plus fréquemment la population agricole que la population générale. L'étude menée, en 1995 par Pierre Lebailly (GRECAN) sur des agriculteurs du Calvados, a montré, par exemple, que le cancer de la prostate touchait particulièrement les éleveurs qui utilisent des produits chimiques mais pas particulièrement des pesticides," explique Jean-Luc Dupupet, responsable du risque chimique à la MSA. Pour lui, "Il est donc important de mener cette étude pour bien cerner quels sont les facteurs susceptibles d'expliquer le nombre élevé de certains cancers en milieu agricole et de déterminer les risques liés à l'utilisation de produits phytosanitaires."

Les résultats sont attendus fin 2008 pour les cancers les plus fréquents (prostate, sein) et pour les populations agricoles où le risque d'exposition est rapidement identifiable. Mais il faudra attendre fin 2015 pour les cancers plus rares (ovaires, lymphomes non hodgkiniens, myélomes multiples) et pour les activités agricoles dont la fréquence d’exposition aux risques est plus difficilement discernable. "Aujourd'hui nous avons des hypothèses, mais on ne sait pas ce qu'il sortira de cette étude. Si elle permet de déterminer des risques spécifiques liés à des produits, nous agirons pour prévenir ces risques. C'est notamment grâce aux études menées par la MSA que l'arsenite de sodium, un cancérogène pour l'homme, a été interdit d'utilisation en 2001," rappelle Jean-Luc Dupupet.

L’étude PARTAGE : Parkinson en milieu agricole

Conclusion de l’étude TERRE (247 cas, 676 témoins), menée en 1998 et 1999 par la MSA et, l’Inserm : le risque d’être atteint par la maladie de Parkinson est multiplié par 1,9 en milieu agricole et par 1,8 pour les personnes longuement exposées aux pesticides. Mais à ce jour, aucune étude ne permet d’incriminer spécifiquement tel ou tel produit ni même de conclure scientifiquement à une relation directe entre la maladie et l’utilisation des phytosanitaires.
C’est pourquoi la MSA, l’Inserm et l’InVS ont souhaité compléter cette enquête en lançant en février 2007 l’étude PARTAGE (Parkinson, Travail, Agricole, Environnement) portant sur 420 cas et 840 témoins. Quatre départements sont concernés : la Charente-Maritime, la Gironde, la Haute-Vienne, et la Mayenne.

Objectifs :

• estimer la frĂ©quence de la maladie de Parkinson en milieu agricole
• comparer le risque de maladie de Parkinson chez les agricoles et les non agricoles
• Ă©tudier la relation entre la maladie et certaines activitĂ©s agricoles
• rechercher les facteurs gĂ©nĂ©tiques Ă©ventuellement associĂ©s

Movember (the month formerly known as November) is an annual charity event held during November. At the start of Movember guys register with a clean shaven face. The Movember participants, known as Mo Bros, have the remainder of the month to grow and groom their Mo, raising money along the way to benefit men's health - specifically prostate cancer and male depression.

On cancer awarness, I understand this month is Breast Cancer Awareness month. Great, fabulous. With this combined with everything that is pink, how could anyone not get the message.

My question is, why doesn't prostate cancer get the same amount or Awareness campaigns as breast cancer does? Yes we have Movember, of which I will be a participant again, but that seems to be all Prostate Cancer gets as an awareness campaign.

Now I realise brown ribbons or brown cricket bat handles (just saw that blue is the colour used - edit) would not be that attractive but I would have thought, men being men, there would need to be a stronger campaign. Men in general do not look for early signs of being unwell and usually only get to a doctor when missing a limb.

I just feel their should be a stronger push to get blokes aware of this and make sure everyone plays a part in Movember..

As many men die from prostate cancer as women die from breast cancer but... a national survey by PCFA in 2002 showed that while 78% of women felt well informed about breast cancer – only 52% of men felt informed about prostate cancer

As October is Breast Cancer Awareness Month, Lifetime, the network for woman and unemployed homosexuals, is campaigning fiercely for awareness of the disease. A commercial informing the viewer that a selection of movies that, I assume, are centered around breast cancer, will be shown to help remind woman that it’s time for a mammogram.

I often have to wonder why it is only the women who advocate such awareness for afflictions that affect them primarily. You can’t go twelve minutes without hearing about cervical cancer, on average less than three minutes without mention of menopause. Meanwhile, men don’t really seem to give a crap about testicular or prostate cancer. If relied solely on what was seen on television, one might assume the only afflictions men dealt with were erectile dysfunction and inadequate penis size. What I want to know is where the male health campaigns are? Where is there a program to mail in beer labels or bottle tops for a cure? Why is there no “Bud Bottles for Balls” awareness program raising money to remind men of legal drinking age that a simple self examination of their testicles can save lives? Why has the SPEED channel not promoted a NASCAR race raising money to fight prostate cancer; why is Richard Petty not on a billboard reminding men to schedule a digital prostate examination?

I’ll tell you why. For some odd reason, most men, excluding myself of course, would rather die than have a doctor sticking his fingers up their butthole. They’re scared they might like it. Yes, I said it! They are scared that first examination is going to end in a seedy bathhouse, strapped in a sling with a fist up their ass smoking meth out of a busted light bulb. I’m not saying it hasn’t happened of course, but I would have to dispute claims that it happens frequently. Just because that’s how I wound up doesn’t mean it will happen to you, I was weak. Learn from my mistakes.

At any rate, the other night I was planning to participate in a BDSM sex scene with a gentleman I had met on the internet. But this one is in an entirely new area.  He is now 76 years old.  They say it can be treated with hormone treatments and specialized radation, so we are hopeful.  He is, of course, going to Fox Chase Cancer Center (FCCC) for a second opinion and maybe a third with a specialist in our area, who used to run that department at FCCC.  I am just very frustrated.  I work for a Doctor of Internal Medicine.  So I have seen this disease run past my desk before.  I know how other doctors have treated it.  I know of additional testing that needs to be done to ensure that it has not spread outside of the area (which they said it has not, but he has not had the testing.)  I keep trying to push my parents to request this testing but feel I am being shushed. 

I know he was just diagnosed yesterday, but if it were me I would be scheduling these tests right away.  Sorry if I am seeming morbid.  I just would want all of the information up front and then we can deal with it.  I just needed to get it off my chest.

Update:  Family listened but doctor still will not do testing I think is necessary.  And life goes on...

Okay, so you might have guessed that my wife and I just had some great sex.  Well, great for her, emotionally satisfying for me.  (Which is to say I didn't have any mindblowing orgasms, but I wouldn't take back that love making for the world.)

As regards  the risk factors for prostate cancer, Wikipedia sums it up nicely: "Prostate cancer presents clinically at an average age of 70years,  although  found incidentally in about 30% of men in their fifties and 80% of men in their seventies. Prostate cancer affected 18% of American men and caused death in 3% in 2005.  Pollution (eg environmental estrogenics, diethylstilbestrol exposure in utero,  Agent Orange) and genes play perhaps 20% to 80% role, which can hardly be prevented by the individual already in midlife:  In Japan, death from prostate cancer was one-fifth to one-half the rates in the United States and Europe in the 1990s. In India in the 1990s, half of those with cancer confined to the prostate died within ten years. African-American men have 50–60 times more prostate cancer and prostate cancer deaths than men in Shanghai, China. In Nigeria, 2% of men develop prostate cancer and 64% of them are dead after two years."  

 Dietary:  blood levels of trans fatty acids, (in particular from hydrogenation of vegetable oils), are associated with an increased prostate cancer risk. Other dietary factors that may increase prostate cancer risk include low intake of vitamin E, vitamin D (incl. sunlight),  fish omega-3 fatty acids , and the mineral selenium." .
